Below is information for parents/guardians of students that attend
Mauriceville Elementary
If your child will be taking any prescription medications at school please have the form filled out and signed by your child’s doctor. You can click on nurse forms link at bottom of page to find this form. Please also complete and sign the bottom portion of the form before turning in to the Nurse’s Office. All prescription medications must have a current label from a pharmacy on the medication (no samples will be given). The label must also be in your child’s name and have dispensing directions that match what the doctor fills out on the form. We will not be able to alter any medication instructions. If your child’s dispensing directions change during the year, a new form will need to be completed.
You are welcome to bring over the counter medications to school for your child to be given if there is a need for such medication. Please fill out the form and return to the Nurse’s Office. You can click on nurse forms link at bottom of page to find this form. It does not need to be signed by a doctor. We are unable to dispense medication without the form on file. Any medication given must be provided by the parent/guardian. We do not keep medications in the office to dispense. All non prescription medications will need to be in their original container and labeled with the child’s name. Please make sure to confrm that the medication is not expired.
If your child has food allergies please have the form filled out and signed by your child’s doctor. You can click on nurse forms link at bottom of page to find this form as well as a severe food allergy packet if your child requires an Epi Pen for their food allergies. We are unable to alter your child’s diet without their doctors consent.
If your child has asthma, a cardiac condition or seizure disorder please have the form and care plan filled out by your child’s doctor. You can click on nurse forms link at bottom of page to find each of these forms.
Illnesses with temperature of over 100.0 degrees will be sent home. Students need to be fever free without fever reducing medication for 24 hours before returning to school.
A student with live pediculosis (head lice) according to policy will be sent home from school. The student may return to school after successful treatment (removal of all live lice). Before returning to class, the student must report to the Nurse’s Office for clearance.
Please keep weather appropriate change of clothes (including underwear and socks) in your child’s backpack at all times for accidents, spills etc. This is especially important for our younger students.
